How to Apply for a Home Care Package
Getting started with a Home Care Package from Trilogy Care is simple. Follow these steps to apply:
Check Your Eligibility
Apply for an assessment with My Aged Care either online at myagedcare.com.au or by calling 1800 200 422. If eligible, the Aged Care Assessment Team (ACAT) will visit your home or contact you over the phone to determine your care needs.
Have Your ACAT Assessment
During the assessment, the ACAT assessor will determine the level of care you need and your priority for receiving a Home Care Package.
Find Out Your Assessment Outcome
My Aged Care will send an outcome letter that includes your eligibility, the services you can receive, and any additional information. You'll be placed in the National Priority System until a package becomes available.
Being Assigned a Package
Once assigned, you'll receive a referral code to access your package. If you choose Trilogy Care as your provider, contact us at 1300 459 190.
Start Self-Managing with Trilogy Care
Once your package is activated, you can begin receiving services through Trilogy Care.

Switching Home Care Package Providers
Switching to Trilogy Care is easy and stress-free. Follow these steps to change your Home Care Package provider:
Call Your Current Provider
Notify them of your plan to transfer and nominate a termination date (typically provide 2 weeks' notice).
Retrieve Your Referral Code
Call My Aged Care at 1800 200 422 (press option 5) or use the online portal to reactivate your referral code.
Contact Trilogy
Care
Call Trilogy Care at 1300 459 190 (press option 1) and provide your referral code to begin the onboarding process.
Home Care Management Options
Self-Managed (15% Fee)
Trilogy Care charges 15% p.a. of the package amount for self-management. Clients can:
- Source and manage their own services and equipment.
- Rely on Trilogy Care for managing financial components, such as payments and reimbursements.
- Work with a dedicated Care Partner to create and modify their care plan and budget.
This option maximizes care hours while allowing recipients full control over who provides their care.
Self-Managed PLUS (26% Fee)
For those seeking more assistance, Self-Managed PLUS charges 26% p.a. and includes:
- A Care Coordinator to manage services and equipment on behalf of the client.
- Ongoing package management, allowing clients to focus on their day-to-day lives.
- Decision-making power for recipients, with services aligned to their preferences.

Getting Started with Trilogy Care
Sign-Up Process
Complete an Online Questionnaire and Sign the Home Care Agreement
The questionnaire collects essential information, including your medical and medication health summary. Upon completion, you'll sign the Home Care Agreement.
Attend a Care Plan
Meeting
Trilogy Care will schedule a care plan meeting over the phone. During this meeting, a customized care plan and budget will be developed.
Once the steps are completed, you can start receiving services and send invoices to Trilogy Care for payment.
