Getting started with a Home Care Package from Dream Big Support Services is simple. Follow these steps to apply:
Apply for an assessment with My Aged Care either online at myagedcare.com.au or by calling 1800 200 422. If eligible, the Aged Care Assessment Team (ACAT) will visit your home or contact you over the phone to determine your care needs.
During the assessment, the ACAT assessor will determine the level of care you need and your priority for receiving a Home Care Package.
My Aged Care will send an outcome letter that includes your eligibility, the services you can receive, and any additional information. You'll be placed in the National Priority System until a package becomes available.
Once assigned, you'll receive a referral code to access your package. If you choose Dream Big Support Services as your provider, contact Dream Big Support Services at (07) 4595 1132.
Once your package is activated, you can begin receiving services through Dream Big Support Services.
